Our Primary Care Networks are working together and are able to provide more appointments to patients across NW London.
The changes mean that practices are able to provide more appointments locally during the hours that best suit patients, this includes evening and weekend appointments. Patients can now book an appointment by calling our Single Point of Access on 07541490200. Patients can also fill out the self-referral form below. 111 online/phone can also book patients in for local appointments in the evening and at the weekend.
Evening and weekend appointments may take place by phone, video, or face-to-face depending on clinical need. Patients may be offered an appointment at their GP practice, another local GP practice or another local NHS service.
These changes mean we can provide more appointments for patients when they most need them.

- Nursing Service: Asthma Review, Child Immunisations, Smears, Spirometry, Pneumovax, shingles
- Health Care Assistant: NHS Health Check, Bloods, Diabetic Review, ECG, blood pressure monitoring, Immunisations, SMI
- Clinical Pharmacist: Medication Review, Asthma and Diabetic Reviews
- Dietician: Dietician Advice
